The CHFNS service is a team of specialist nurses skilled in the management of patients with heart failure. This service operates Monday – Friday, 9am-5pm. The team are offered clinical support and guidance by local Cardiologists at the Whittington, University College London Hospital (UCLH) and North Middlesex Hospital (NMH). The heart failure referral form is available on the right side of page.

The CHFNS will:

  • Only accept patients with a confirmed diagnosis of heart failureThe diagnosis must be confirmed by Echo & the patient must have been seen by a Cardiologist or a physician with a specialist interest in heart failure in line with NICE guidelines. This information must be provided on referral to the CHFNS.
  • See patients within 2 weeks post Discharge from Hospital following a heart failure admission
  • Offer early review to those patients already known to the service, if they deteriorate and need to be reviewed

Patients with an unconfirmed diagnosis of Heart failure should be referred to the Rapid access Heart failure Clinic at Whittington, University College London Hospital (UCLH) and North Middlesex Hospital (NMH).

The CHFNS will provide regular follow up to ensure patients are stable and on optimal heart failure medication. During this time they will provide ongoing education and support, so the patient can self manage their condition. Once the patient is stabilised they will be referred back to GP for routine care.
