Anticoagulation Service: UCLH

NCL Wide

The Anticoagulation service at UCLH comprises of a nurse-led clinic supported by a multi-disciplinary team. The clinic initiates and monitors patients on vitamin K antagonist (e.g. warfarin) and the initiation and review of patients on direct oral anticoagulants (DOAC).

Patients who have been initiated on warfarin should have been provided with an NPSA oral anticoagulant therapy record book (see Downloads) by the hospital clinic. This booklet should be taken with them to any hospital/GP appointments and when collecting anticoagulant prescriptions.


Eligibility criteria

Inclusions

  • All adult patients in need of anticoagulation and those already on anticoagulation that requires monitoring

Exclusions

  • Children
  • People not on or requiring anticoagulation
