In preparation for the autumn Covid-19 booster campaign, the national team has provided consent form templates that can be used by practices and teams to obtain consent for residents to be vaccinated, and for staff vaccinations. As the autumn booster is not part of the primary course, new consent is required. 

These templates may be useful in obtaining consent from residents or a relative/attorney if the resident lacks capacity, or sites may wish to continue using locally established processes for obtaining and documenting consent.

Feedback relating to the spring booster rollout was that obtaining consent was a key barrier faced by PCNs and providers, which led to delays in visits taking place at care homes. To overcome these challenges, it is hoped that work to obtain consent can begin now in order to give enough time to support any care homes that are facing particular issues. This will enable partners to support these homes and resolve issues so that visits can be planned in time for the start of the autumn Covid-19 booster campaign.
